Primary Responsibilities
Under the general supervision of the Director of Pharmacy, delivers pharmaceutical care consistent with the department's clinical role in assuring rational drug therapy in accordance with the department mission statement and department policies and procedures.

Under the general supervision of the Director of Pharmacy, compounds and dispenses medication in accordance with the department mission statement and department policies and procedures.

Major Duties and Responsibilities

Develops, evaluates, recommends and monitors patient pharmaceutical care plan:
Assists prescribers in developing optimal drug therapies.
Monitors drug therapy.
Performs pharmacokinetic evaluations.
Collaborates with prescribers and caregivers to improve patient medication outcomes.
Participates in development and implementation of medication pathways and protocols.
Provides patient and family medication education.
Ensures an accurate patient medication history.
Helps assure continuity of discharged patients pharmaceutical care.
Performs long term care consultant pharmacist duties.

Receives, reviews and interprets medication prescriptions:
Uses computer medication profiles to ensure that dosing is appropriate, and that adverse drug interactions and therapeutic duplicates or incompatibilities are avoided.
Assists with rounds to patient care units to pick up orders and deliver medications.

Consults and confers with prescriber and caregiver:
Clarifies prescription with prescriber or caregiver as appropriate.
Alerts prescriber and caregiver to unusual or dangerous drug therapies.
Contacts and confers with prescriber whenever prescription irregularities arise.
Confers with prescribers and caregivers concerning availability, form, side effects, and drug interactions of medication prescribed.
Performs pharmacokinetic consultations.

Dispenses and compounds prescribed medication:
Obtains patient clinical information, performs computer order entry, ensures appropriate product selection.
Compounds pharmaceutical preparations as necessary, using standard pharmaceutical physical/chemical procedures and equipment.
Performs related pharmaceutical mathematical calculations.

Serves as clinical and drug information resource to prescribers, caregivers and patients:
Participates in the department teaching, training, and competency activities.
Precepts student externs and student interns.
Prepares and presents staff, caregiver, prescriber, community education programs.

Documents and maintains patient and department records:
Including: purchasing; controlled substance records.
Participates in department inventory taking and reporting activities.

Supervises work of supportive personnel:
Performs duties as charge pharmacist in coordinating and supervising the supportive personnel in the daily operational functions and activities.

Performs related and assigned duties as required:
Assists in performing drug expiration date inspections/replacements.
Assists with, or may have full responsibility for special projects as assigned by the Director of Pharmacy.
Additional duties may include, but are not limited to: quality assurance/performance improvement activities; newsletter publication; committee assignments.

Job Requirements
BS degree in Pharmacy
Completion of one year internship
Licensure or eligibility for licensure as a Registered Pharmacist in the State of Maine
Must undergo orientation/training period
Hospital pharmacy experience is preferred but not required

Ability to provide age appropriate pharmaceutical care to neonatal, pediatric, adolescent, adult, and geriatric patients.

Ability to maintain confidentiality of information.

Special Skills:
Ability to work with computers (mainframe & personal) and software (spreadsheet, word processing, database).
Ability to operate machine, tools, and work-aids of the department.
Ability to adapt to a changing work environment.

Normally an eight to ten hour shift.
Rotating shifts, including evenings and weekends.
Rotates through department on-call schedule as required.
Rotates to different division work sites as required.
Periodic extra time may required.
Periodic callback may be required on both standard and non-standard shifts.

Physical Demands:
Sitting – 3-7 hours per day (generally less than 25% of the time).
Standing and Walking – 3-7 hours per day (generally more than 75% of the time).
Lifting - moderate lifting of 10lb to 50lb with good body mechanics.
Twisting – 1-120 twists per day.
Bending – 1-20 bends per day.
Squatting or Kneeling – 1-3 hours per day (generally less than one hour per day).
Wrist Position – Moderate deviation of the wrist.
Pinching or Grasping - moderate to frequent (121-480 pinches per day).
Hand/Wrist Repetitions - moderate to frequent (961-2280 per day).
Manual Dexterity - frequent fine motor movement.
Either Handedness - up to 60% of the time.
Both Handedness – between 36-60% of the time.
Near Vision - requires very accurate near vision.
Far Vision - requires 20/40 far vision.
Visual Color Discrimination - requires discriminating between red, green, and white.
Hearing – requires hearing whispered voice at 8 feet (more than 75% of the time).
Speech - frequent clear speaking ability required.

Work Environment:
Moderate to frequent exposure to 80-90 degree temperatures.
Frequent exposure to stressful, crowded working conditions.
Frequent exposure to steady equipment and personnel noise, generally below 85db.
Slight or potential exposure to toxic and/or caustic drugs and chemicals.
Slight or potential exposure to patients and/or staff with infections.
Moderate exposure to injury from sharps.
