Population Health Coordinator - Quality Admin

POSITION SUMMARY: The Population Health Coordinator will work with the Director and Primary Care providers to address education, workflow and other barriers to the overall health and wellness of the patient populations. This includes chronic disease management, health screenings and EMMC's own population health initiatives. The Coordinator will identify patients who are not meeting specific quality measures and work with providers and practice staff to ensure the patient is compliant and quality measures are met and improves the overall care of the patients through sustained workflow improvement. The Coordinator works with other members of the Medical Group Quality Team and the practice staff to reach mutual goals for quality of care and patient safety.
MINIMUM EDUCATION REQUIRED: Associates Degree
MINIMUM EXPERIENCE REQUIRED: 5-years in Healthcare
- Bachelor's degree and/or 5-years in healthcare physician office setting strongly
- Work experience in healthcare office quality/performance improvement or primary care office setting.
OTHER SKILLS REQUIRED: Computer proficiency including Outlook, word processing, database, and spreadsheet software. Ability to dissect issues in datasets, address the opportunity with key clinical staff and cater education improvements to match the needs of the care team. Maintain an active program of self -study and learning to enhance professional skills and to stay current with technological changes to enhance department objectives. Ability to read and interpret documents. Ability to communicate with wide variety of hospital personnel at all education and job levels and maintain professional attitude and high level of discretion. Ability to listen well and collect data. Ability to solve practical problems. Ability to recognize need for assistance and boundaries of responsibilities and authorities.

PHYSICAL REQUIREMENTS: Ability to perform multiple office work functions with keyboard/computer, telephone, and other usual office equipment. Extensive reading and paper handling/filing. Sufficient mobility to attend frequent meetings both on and off campus and assist in quality/performance improvement activities. WORKING ENVIRONMENT: Busy primary care office environment, frequent interruptions, frequent interaction with coworkers, physician and nursing leadership, practice staff and administrative leaders.
