Performs telephonic health education based on participants completed health risk assessment and lifestyle priorities; help clients to set both short and long term goals; and monitor progress towards goal attainment. The coach will provide positive reinforcement, advice, and support. Performs an interface between APS and Health Plan members, regarding program components, the members, and the provider community.

Position is based out of either South Portland or Bangor, and requires up to 25% travel.

Responsibilities:
1. Provides telephonic outreach to health/wellness health plan members.
2. Supports healthy lifestyle choices that will reduce long term effects of illness and improve quality of life.
3. Delivers ongoing education and outreach for participants on wellness program strategies in conjunction with customer health event, sponsored events, and programs. Requires extensive local, in state, travel with minimal overnight travel.
4. Collects clinical data and enters information into database for tracking and metric purposes.
5. Develops education and informational materials for Health Plan participants and providers as necessary and prepares summary reports as needed.
6. Maintains open communications with the customer to coordinate outreach activities as necessary and appropriate.

Education:
Licensed Professional (LCSW, LCPC, MSW) or BA/BS in Behavioral health, Dietetics or related Health Care Field; BS prepared and certified/credentialed/regulated by the State in which they provide services.
RN preferred

Requirements:
Minimum 3 years clinical experience.
Previous experience in health coaching preferred.
Clinical background, preferably in quality, technology, or performance improvement.
Knowledge of insurance or managed care preferred.
Understanding of disease management and health and wellness industry preferred
Knowledge of job specific medical specialty (disease management/chronic illnesses/maternity care).
Clinical assessment skills, consistent with state licensure scope of practice requirements.
Excellent written & verbal communication skills.
Telephonic interviewing skills.
