Summary of Duties:
The Community Health Staff Social Worker at Mid Coast Health Services is a medical
social services position. The social worker assists the RN and other team members in
understanding the significant social and emotional factors related to the patient s health
problems. The Social Worker is responsible for the assessment, planning, implementation and evaluation of patient s social and emotional needs. Demonstrates positive community relations through effective communication skills, patient advocacy and maintaining collaborative relationships using a team approach. Respects and values socio-economic and cultural diversity. The Community Health Staff Social Worker is expected to meet standards of care and contribute to improvements in quality of care. Supports departmental policies and assumes responsibilities for meeting professional education goals. This level is accepted as the standard of practice at Mid Coast Health Services/CHANS.

Skills Required:
Masters degree in social work from an accredited college
Eligible for Maine state licensure
One year social work experience in related area; community health experience preferred
